Care built around one person at a time

Every service below can be combined, scaled up or scaled back as needs change. Nothing here is a fixed package.

In-Home Behavioral Care

Behavioral Support

Tailored behavioral services that foster understanding and enhance quality of life for individuals and their families. We work in the environment where behavior actually happens — the home, the workplace, the community — so that progress holds up in real life.

  • Person-centered behavioral planning and goal setting
  • De-escalation and coping strategies taught to the whole household
  • Family and caregiver coaching, so support does not stop when we leave
  • Regular documentation and review with your case manager

Daily Living & Personal Care

Adult Health Care

Extensive assistance for adults, fostering dignity and independence in everyday life. This includes adults with disabilities seeking to reside outside their family home — whether in group homes or supportive living environments — as well as elderly clients who want to stay in the house they love.

  • Personal care, mobility assistance and daily living support
  • Medication reminders and appointment coordination
  • Transportation support for clients who struggle to reach clinics and hospitals
  • Meal preparation, light housekeeping and household routine
  • Wellness checks and family status updates

Skills, Employment & Community

Guided Independence

Our mission is to assist individuals in living autonomously, acquiring skills, and discovering purpose through employment, community involvement, and everyday experiences. Independence is not a milestone we hand over — it is something we build together, one skill at a time.

  • Life-skills coaching: budgeting, cooking, transport, self-advocacy
  • Employment readiness and workplace support
  • Community integration and social connection
  • Support for the transition to supported or independent living

Transition & Adjustment Support

Assisted Learning to New Life Phases

Comprehensive support to adults adjusting to a life with more dependencies. A new diagnosis, a hospital discharge, the loss of a spouse or the loss of a license — these moments change what daily life looks like. We help people find their footing again without losing their sense of self.

  • Transition planning after hospital or rehab discharge
  • Routine rebuilding and adaptive strategies for the home
  • Emotional support and counselling connections
  • Family education so everyone knows what to expect

Community-Based Support

Behavioral Support Beyond Home

Enhancing lives through individualized behavioral understanding — wherever life takes place. Care that stops at the front door is not really care, so our support follows clients into the appointments, activities and community settings that matter to them.

  • Accompaniment to medical and therapy appointments
  • Support in day programs, workplaces and community settings
  • Coordination with clinicians, schools and case managers
  • Consistent strategies used by everyone on the care circle

Family Respite & Companionship

Respite & Companion Care

Family caregivers need care too. Respite gives you a scheduled, dependable break — a few hours a week or a longer stretch — knowing your loved one is with someone who already knows their routine. Companion care addresses the quieter problem: isolation.

  • Scheduled respite blocks for family caregivers
  • Companionship, conversation and shared activities
  • Errands, appointments and community outings
  • Consistent caregivers so trust has time to build
